Note that older versions of the game had a bug that made it possible to get the lighthouse scene with the "let's try again" option, but that has been fixed.
To romance Triss, you need to (obviously in addition to not doing the above) complete A Matter of Life and Death, kiss her, then complete Now or Never and at the docks say "stay with me" and then "I love you". Anything else, even the unicorn scene, does not matter. You romance Yennefer only if you complete The Last Wish and say at the end that you still love her. So, in the worst case you need a save from Final Preparations (before meeting the Lodge), otherwise from Battle Preparations before going to Avallac'h's lab.
do not let Ciri destroy Avallac'h's lab meet the Lodge with Ciri (rather than letting her meet them alone) accept the money from Emhyr for bringing back Ciri (if you visit the emperor before Bald Mountain)
drink with Ciri after The Battle of Kaer Morhen (instead of having a snowball fight) You get the bad ending if you make at least 3 of the following choices: That means you probably got the bad ending (because of choices made in conversations with Ciri), which is not related to The Last Wish.
